Create a heading (+ ... through ++++++ ...) container.
Parameters
- level: HeadingLevel
  Heading depth (1-6)
- elements: Element[]
  Heading content
- hasToc: boolean = true
  Whether to include in the table of contents (default: true)
- attributes: AttributeMap = {}
  Optional HTML attributes
